I hereby grant permission to the rights of my image, likeness and sound of my voice as recorded on audio or videotape without payment or any other consideration to Kairos Excellence PC or The Source Chiropractic Franchise PC.  I understand that my image may be edited, copied, exhibited, published or distributed and waive the right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of my image or recording.  I also understand that this material may be used in diverse settings within an unrestricted geographic area.  

By signing this release I understand this permission signifies that photographic or video recordings of me may be electronically displayed via the Internet or in public settings.

There is no time limit on the validity of this release nor is there any geographic limitation on where these materials may be distributed.


Participation Release:

In consideration of being allowed to participate in any way in Energy Camp, Energy Somatics, related events and activities, coaching, modules, certifications and lessons, the undersigned acknowledges, and agrees that:

1) There is a potential for risk of injury

2) I knowingly and freely assume all risks both known and unknown, whether arising from the negligence of Energy Somatics, Brett Jones, Kate Jones, Kairos Excellence PC., The Source Chiropractic Franchise PC, or their staff, or presenters, or from that of myself or others, and assume full responsibility for my participation; 


3) I willingly agree to comply with the stated and customary terms and conditions for participation. 

 

4) If I observe any hazard during my presence or participation, I will remove myself from participation and immediately bring such hazard to the attention of the nearest facilitator and,

I hereby assume all risk of injury or harm as a result of the activities specified above and agree  to release, indemnify, defend, and forever discharge the Energy Somatics,  Brett Jones, Kate Jones, Kairos Excellence PC.  their officers, facilitators, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event from all liability, claims, demands, damages, costs, expenses, and causes of action due to death, injury, loss, or damage to the undersigned.

I agree to not distribute, allow access, copy, forward, or replicate any of the content for free, monetary gains, or trade. Any and all redistribution of program content is strictly forbidden without prior written authorization from Energy Somatics, Kairos Excellence PC, The Source Chiropractic Franchise PC or Brett Jones.

 Release of Liability and Restrictions on Use of Intellectual Property

I acknowledge that the education provided in these modules and this in person event is strictly reserved for the application, learning, and development of healing facilitators holding a professional license, chiropractic students, and licensed chiropractors. I acknowledge that the teaching and information that I receive through this event does not qualify or certify me to in the Energy Somatics work.  I agree to not implement these teachings if 1) I am not a licensed chiropractor or 2) a student enrolled in a chiropractic training program unless I am being supervised by a licensed chiropractor.  OR 3) a healing facilitator with proper professional credentials and/or licensure to facilitate energy work for clients in a professional setting,


I hold harmless Kairos Excellence PC. and The Source Chiropractic Franchise PC., their mentors, affiliates, Brett Jones, the presenters, guest educators (hereafter referred to as “Kairos”) from any use of the content of these modules.  I acknowledge that this content is for educational purposes only and does not provide me with a degree or the credentials necessary to practice chiropractic without a professional license or Energy Somatics work without the proper credentials/certification.
